2013 Grant Guidelines
Deadline: All Grant proposals must be postmarked on or before March 15, 2013 with one original and five (5) copies mailed to: Sheila Collins, Grant Committee Chairman The John E. Fogarty Foundation One Turks Head Place Suite 205 Providence, RI 02903 Or they may be emailed by the above deadline to: info@fogartyfoundation.org.
Grant Awards Ceremony: A ceremony will be held late May/early June in Providence, Rhode Island. All Grant recipients will be notified two (2) weeks prior to the date. A member of your organization should be present to receive the award.
Applying for a Grant
Grant proposals must follow the application format (attached here) and include all requested information. Only complete proposals will be considered for funding. The Grant Committee considers many factors in its deliberations among them are:
- Submission of Progress Report – Past recipients will not be considered for funding unless a Progress Report for the most recent award has been filed. The deadline for Progress Reports is February 15, 2013 They may be sent to Sheila Collins at the above address or emailed to: info@fogartyfoundation.org.
- Significance of the services on the population to be served, the number of people served and ways in which proposal promotes inclusion.
- Is the Fogarty Foundation Grant critical to the proposal? Can the program and/or organization be sustained?
- Is the request innovative – if so, how?
- Can the program be replicated and/or shared?
- Detailed project budget.
If you are submitting a grant please provide the following information:
- Narrative
- Background– This should describe your organization including the following:
- Current progams.
- The number of full and part-time staff.
- List total amount of your organization’s budget for 2012 and the date for the end of your fiscal year.
- Percentage of budget that covers general operating expenses.
- Description of your constituents: including total number and age range.
- Request for Funds– Please address the following:
- Specify the goals of your project.
- Describe the population to be targeted and the number of people who will benefit from the project.
- Discuss how the proposed project will promote inclusion of people with intellectual and development disabilities.
- Provide a timetable for meeting the goals of the project.
- Discuss your plans for funding and sustaining the program. In this section, please list other agencies and organizations that help you with funding for this project.
- Indicate how you will measure the success of this project. What are your desired outcomes? How will you know whether your outcomes have been attained?
- Background– This should describe your organization including the following:
- Attachments
- Financial Information
- IRS 501 (c)(3) Tax Determination Letter
- A list of public and private sources of funding for the past fiscal year. Individual donors need not be included. For private sources please list entity or foundation if support is equal to or greater than $5,000.
- Comprehensive project budget including all expenses and other sources of income for the project both committed and prospective with amounts. (Please attach project budget separately not to exceed two pages in length).

- List of Board of Directors/and or Trustees.

- Financial Information
Incomplete applications will not be considered for funding
